외상에 의한 급성 경막하혈종이 수술적처치없이 조기에 자연소실을 보인 1예를 경 험하였다. 환자는 31세 남자로 처음 시행한 CT에서 약 9mm 두께의 급성 경막하혈종이 좌 측 측두골에서 관찰되었으며 제 11-12흉추의 골절탈구에 의한 하반신 마비를 보였다. 환자 는 급속히 의식상태가 개선되어 4시간후 추적 CT를 시행한 결과 혈종의 크기가 상당히 감 소하였음을 알 수 있었다. 저자들은 이러한 급성 경막하혈종의 조기 자연소실의 임상경과와 기전을 문헌고찰과 함께 보고하는 바이다.
We present a case of acute subdural hematoma which was rapidly resolved without surgical intervention. This 31-year-old man had a hematoma of which thickness was 9 mm and was paraplegic due to fracture-dislocation of thoracic spine at the level of Tll-12 Rapid recovery of consciousness despite of sizable hematoma made to take a serial CT scanning instead of immediate surgical intervention. The hematoma was resolved within 4 hours without surgery. Possible mechanism of this rapid spontaneous resolution is discussed with brief review of the related literature.
