상악 전치부는 구치부에 비해 상실 초기부터 환자의 심정 적 반응이 크게 수반되는 부위이다. 상악 전치부가 소실된 환자는 결손에 대해 저작이나 발음의 문제 이외에도 자신의 외모 변화에 대해 민감한 반응을 보인다. 따라서 환자는 가 능한 한 빠른 시간 내에 전치부의 수복을 원하며 이에 그 동안 조기 수복에 대한 많은 연구가 있어왔다. 다행히 그 동안 임프란트 표면에 대한 기술이 나날로 발 전함에 따라 즉시 부하가 가능할 정도로 초기 안정성(primary stability)과 빠른 시간 내에 이차 안정성(second stability)을 획득 할 수 있게 되었다. 상악 전치부 102개의 단일치 임프란트에 대한 전향적 연구에서 Kemppainen 등 은 1-stage와 2-stage 임프란트에서 99%의 성공률을 발표 했다1).
The restoration of maxillary anterior tooth by implant is difficult that most patients have trouble with chewing, prounciating, even more aesthetic. Fortunately there are many technological advances in implant surface, so we can get a early primary stability and second stability. but atrophy is most severe after restoration in the anterior maxilla with the degree of horizontal/vertical bone/gingival resorption. The highly esthetic zone of the premaxilla often requires both hard (bone and teeth) and soft tissue restoration. This is the case we performed delayed implant installation with Guided bone generation (GBR) for the purpose of preventing bone resorption , connective tissue graft to change gingiva from thin biotype to thick biotype and zirconia abutment and all ceramic crown to prevent gingival recession after restoration.
