실신을 주소로 내원한 64세 여자 환자에서 심 초음파상 좌심방 내에 $4.2{ imes}2.4;cm$의 다혈관성 종괴가 발견되었으며, 좌심방 혈관종 혹은 점액종을 의심하여 종양 절제술을 계획하였다. 술 전 시행한 관상동맥 조영술상 좌회선지에서 기시하여 종양으로 분포하는 내경 2 mm의 영양혈관이 발견되었다. 이에 종양 절제술을 시행함과 동시에 좌회선지에서 기시하는 영양혈관을 심외막에서 지혈클립으로 이중 결찰하여 좋은 결과를 얻었다.
A 64 year-old woman presented to out hospital because of syncope. Transesophageal echocardiography showed a $4.2{ imes}2.4;cm$ hypervascular mass in the left atrium. We assessed the mass to be a myxoma and we planned to excise the mass. The preoperative coronary angiography showed a feeding artery with an inner diameter of 2mm originating from the left circumflex coronary artery, so we excised the mass and clipped the feeding artery with two clips at the epicardium with a good result.
