본 연구는 고령화와 더불어 노인 만성신부전 질환자가 급증하고 있는 반면, 기존의 의료서 비스 중심의 전략이 주된 노력이라는 문제점과 이들에서 발생하는 다양한 욕구를 대응할 수 있는 사례관리 실천 적용의 필요성에 주목하였다. 본 연구의 대상자로는 경기도에 위치한 S종 합병원에서 의료사회사업 팀에 의뢰된 신장장애 여성 노인 환자로 의료 기관과 지역사회 사 회복지 서비스 관련 기관들이 협력하여 통합적 사례관리를 진행하고자 하였다. 사례 진행에 대한 전반적인 소개와 개입의 효과를 점검해보았다. 연구방법으로는 단일사례설계를 적용하였 다. 연구 결과, 질병 진행과 관리, 그리고 생․심리․사회적 기능이 향상되었다. 구체적으로는 신장질환의 진행 악화가 예방되었고 신체적 기능이 향상되었다. 우울감이 감소되었으며, 지역 사회 사회복지 자원과 자원봉사 자원 연계가 증가하여 사회적 관계망이 증가되었다. 요양보호 자를 연계하여 약물 관리와 자가 관리 역량을 지원하도록 간접적인 영향을 미치는 성과 가져 왔다. 연구 결과를 기반으로 신장장애 노인 대상 통합사례관리에 대한 과제와 향후 개입 방향 을 제시하였다.
Currently chronic kidney disease among elderly population has been increased with various needs from preventing acceleration of kidney disease to satisfying quality of life. The comprehensive case management service for this population is urgently needed but few expert addressed this issue. Thus, this study was to explore the potential to employ the comprehensive case management service by providing medical and social services to a female client with kidney disorder and her family. Medical social work service team in S hospital located Seongnam Si was involved in this case from September to December, 2014. This study employed the single case study design to evaluate the medical and physical function, psychological function, social network, and medication and self management skills with standardized instruments(quality of life, depression), and the rest of other dimensions assessed by medical records and goal-attainment scales. Based on the results, the discussion and future challenges were addressed.
