본 연구는 일개 상급종합병원에서 입원 및 응급실 치료를 받은 환자 중 진료협력팀으로 의뢰된 환 자의 전원상담 현황을 분석하여 전원대상자의 특성 및 상담관련 특성을 파악하기 위한 후향적 조사 연구이다. 연구대상은 2008년 1월부터 2015년 12월까지 퇴원계획상담 기록지가 작성된 퇴원환자 31,329명의 전원관련 의무기록 내용으로 선정하였다. 연구결과, 진료협력센터에 전원상담 의뢰하는 건 수는 지속적으로 증가하고 있다. 상담 결과 약 70%가 전원을 진행했으며, 특히 응급실 환자의 경우 입원환자에 비해 전원비율이 높았다. 세부적으로는 진료과, 진단명, 의료보장형태, 퇴원 시 거동유무 등에 따라 전원상담 후 전원하는 의료기관종별에 차이가 있었다. 진료협력센터 간호사는 환자의 질병 특성 및 환자의 상태를 고려하여 신속한 전원관련 정보 및 서비스 제공이 요구된다. 본 연구는 상급 종합병원 환자의 전원상담 현황 및 특성을 파악한 점에서 연구의 의의를 지니며, 향후 지역을 확대하 고 더 많은 의료기관을 대상으로 하여 실제 전원현황이 어떻게 되고 있는지 파악을 위한 연구가 필 요할 것으로 사료된다.
This study is a retrospective study to analyze the characteristics of refer consults and the characteristics of counseling patients by analyzing the current counseling status of the patients who were referred to the Referral Center among the patients who were admitted to the general ward and the emergency room in a tertiary hospital. The subjects of the study were 31,329 patients who were discharged from the hospital between January 2008 and December 2015. As a result, the number of requests for consultation to the Referral Center has been steadily increasing. The percentage of referred patients in the emergency room was higher than that in the hospitalized patients. There was a difference in the types of refer medical institutions such as the medical department, diagnosis name, medical care type, and whether or not to walk. The nurse of the Referral Center is required to provide information and service quickly considering the characteristics of the patient and the condition of the patient. This study is meaningful in that the present status and characteristics of the patients' counseling in the Tertiary Hospital Referral Center are grasped and it will be necessary to expand the area in the future and to study the current status of the medical institutions.
