본 연구는 우리나라 장애인복지관의 부설 내지는 조직의 일부로 운영되고 있는 재가복지사업 영역의 지역사회 중심 재활(Community Based Rehabilitation, CBR) 실천의 역사와 현재에 대한 비판적인 검토를 토대로 장애개념 변화에 부합하는 그리고 장애인의 주체적인 참여를 강조하는 새로운 방식의 실천 모형을 제안하는데 목적이 있다. 전통적인 CBR 접근은 의료모델에 기초하고 있다는 점과 의료자원이 빈약한 개발도상국의 상황에 기초한 실천 개념이라는 점에서 비판의 대상이 되고 있다. 그럼에도 불구하고 CBR은 장애인 서비스의 지역사회에 기초한 접근을 강조하고 견인해 왔다는 점에서 그 유용성도 여전히 인정되고 있는 것으로 보인다. 이런 점에서 CBR의 전통적인 접근은 장애개념의 변화에 부합하면서 일정 정도 장애인에게 필요한 의료서비스가 공급되고 있는 국가들에서도 적용될 수 있는 새로운 구조와 전략을 모색해야 하는 시점에 있다. 이런 시점에서 우리나라 장애인복지관의 CBR 실천은 장애개념 변화와 관련된 CBR의 새로운 개념 구성에 대한 세계적인 논의를 반영하여, 이 분야의 논의와 실천에 새로운 활력을 찾아야 하는 과제에 직면해 있는 것으로 보인다. 이를 위해서는 CBR의 재 개념화에 대한 논의가 필요하며, 외국의 선행연구들은 지역사회중심의 ‘재활’에서 지역사회중심의 ‘지원’으로 실천준거가 변화되어야 함을 지적하고 있다. 이에 본고에서는 전통적인 CBR 접근의 핵심적인 요소였던 지역사회조직사업과 사례관리에 더하여 장애개념의 변화에 따라 장애 당사자의 주체적인 참여를 강화한다는 측면에서 장애인의 자조조직화를 추가적인 핵심요소로 제기하였다. 그리고 이들 각 요소들이 유기적으로 결합될 수 있도록 촉진하는 방안으로 구조화된 집단프로그램의 도입 방안을 모색하였다.
The purpose of this study is to suggest the new approach of the Community Based Rehabilitation(CBR) practice in the Community Rehabili tation Center. For this, the history and present of Korean context related to the area of home visiting service were critically reviewed. Traditional CBR approach has been on the object of criticism, because it was based on medical model of disability and rooted in the conditions of developing countries. Nonetheless, this approach has been perceived as useful practice which was facilitating community based approach rather than institutional one. So, CBR practice is in the need of exploring the new structure and strategies which are consistent with the alternative concept of disability and conditions of developed countries in the medical system. Considering these overall situations, we should think over new direction on CBR practice. It could be Community Based Support(CBS) instead of Community Based Rehabilitation(CBR). Traditional CBR practic e included two major strategies, which were community organizing and case management. This study proposed that traditional CBR practice should be changed by adding two strategies, which are the support on the self-help organization of disabled people and the development and implementation of structured group training program.
