본 연구는 주의력결핍과잉행동장애(ADHD) 아동들과 주의력결핍과잉행동장애와 일과성 틱장 애(Transient tic disorder: TTD)가 공존하는 아동들의 신경심리기능, 특히 실행기능의 특성을 비 교분석하고자 하는 목적으로 시도되었다. ADHD로 진단된 아동들 13명과 ADHD+TTD로 진 단된 아동들 13명을 대상으로 연령, 성, 지능을 대응표본하여 인지기능, 주의력, 실행기능과 관련된 신경심리검사를 시행하였다. 검사도구는 전반적인 인지 기능을 평가하거나 장애를 진 단하기 위한 정보를 얻기 위해 폭넓게 사용되고 있는 Wechsler 아동용 지능검사(K-WISC-Ⅲ), 주의력을 측정하기 위해서 전산화된 주의력장애진단시스템(ADHD Diagnostic System: ADS), 실 행기능을 평가하기위해서 아동색선로검사(Children's Color Trails Test: CCTT)와 아동색상단어검 사(Stroop Test; STOOP)를 실시하였다. 그 결과, 전체지능을 대응 표본한 ADHD 집단과 ADHD+TTD 집단 간에 지능검사 소검사 프로파일 비교에서 ADHD 아동들이 산수 소검사 수행이 유의미하게 저조한 것으로 나타났다. 주의력과 실행기능에서는 ADHD와 ADHD+TTD 아동들이 정상 아동들에 비해 부진한 수행을 보였으나 집단 간에 유의미한 차이는 나타나지 않았다. 이러한 결과는 ADHD 집단에서 TTD의 공존으로 실행기능에 더 큰 어려움이 야기되 는 것이 아님을 시사해준다. 끝으로 연구의 의의와 제한점이 논의되었다.
This study was conducted to investigate differential characteristics of neuropsychological functions, especially in executive functions in children of Attention deficit hyperactivity disorder(ADHD) with and without Transient Tic disorder(TTD). Two groups of children(ADHD-only, ADHD with co-occurring TTD) matched for age, sex and IQ(n=13 each). Neuropsychological tests to measure cognitive function(K-WISC- Ⅲ), attention(ADS), and executive function(CCTT, Stroop) were individually administered to 26 children. And the estimated scores of those tests were calculated based on the age norm for each test. In result, there were significant difference at the Arithmetics subtest of K-WISC-Ⅲ. ADHD group tended to show lower scores at the subtest of Arithmetics. In the test of the attention and executive function, ADHD and ADHD with TTD groups demonstrated similar performance patterns and deficits compared with the normal group. This findings suggested that TTD did not impose additional executive function impairments on ADHD. The clinical implication and limitations of the present study were discussed.
