내이의 유모세포나 그 주위 청신경 경로에 여러 가지 원인으로 기능상의 문제가 있을 경우에 기존의 보청기(hearingaid)로는 청력재활이 사실상 불가능한 경우들이 많다. 이러한 환자의 청력재활에 인공와우(Cochlear Implant) 시스템이 도움을 줄 수가 있다.1). 인공와우 시스템은 근본적으로 전기자극을 통하여 청신경을 자극함으로써, 청력재활을 시도하는 방법이다. 일련의 과정을 거쳐서 음압이 전기신호인 활동전위(action potential)로 변환되어 대뇌로 전달된다는 사실이 이 방법이 가능한 이론적 근거이다.7) 이 인공와우 시스템에서 전기자극에 의한 청각 재활이 가능한 원리 및 그 효과에 영향을 미칠 수 있는 기본 언어처리방식(coding strategy)을 분석하였다.
The speech coding strategy is one of the important parts influencing the performance of the cochlear implant system. This process extracts crucial features in speech sounds and processes electrical versions of them to stimulate residual auditory neurons effectively. The nature of strategies can be classified as pulsatile, analog, and mixed waveform stimulations depending on waveform morphologies. The number of channels at the time of stimulation determines the modes of simultaneous, sequential, and mixed time base stimulations. The modes of stimulation can, even, be mixed as in electrical and acoustical stimulation (EAS). There have been diverse variations and the corresponding names of coding strategies for these stimulations. Currently, new types of coding strategies keep emerging: some are available now and others are waiting for clinical evaluations. Developing effective coding strategies seems to involve evaluating the performance in clinical trials based on auditory neural response data to the electrical stimulation.
