난청인 특히 역동범위(dynamic range, DR)가 좁은 감각신경성 난청인에 대하여 보청기의 이득(gain)과 주파수 반응곡선(frequency response curve)을 결정하는 것은 매우 까다로운 편이다. 일반적으로 이득의 결정을 위해 사용하는 청각적인 요소에 역치(hearing threshold level, HTL), 쾌적수준(most comfortable level, MCL) 등이 있으나 MCL의 경우는 측정방법, 신호의 종류 등에 따라 측정값의 변동 폭이 크기 때문에5)12) 보청기 처방법(hearing aid prescription method)은 주로 HTL을 이용하고 있다. 보청기의 효과적인 이득을 결정하기 위해 선형(linear type) 보청기의 경우는 HTL의 1/2 값을 이득으로 정하는
A reverse-sloping sensorineural hearing loss is one of the difficult cases to obtain satisfaction with amplification. This type of hearing loss typically involves greater 2-cc coupler gain for the high frequencies than for the low frequencies based on the hearing aid prescription methods. However, too much gain in the low frequencies would cause many problems including occlusion problems, amplification of background noise, and decrease in speech intelligibility. We tested two individuals with reverse-sloping hearing losses who preferred much less gain in the low frequencies than in the high frequencies. The subjects preferred less gain at low frequency than gain measured by NAL-NL1 method. Based on the setting above, improving tonal quality and speech discrimination was achieved for a reverse- sloping sensosineural hearing loss.
