뇌졸중은 우리나라에서 단일 질환으로 남녀 모두 사망률 제1위를 차지한다. 또한 생존한다고 하여도 여러 유형의 신체 장애와 통증이 나타나며, 우울 등 감정 장애를 경험하는 것으 로 보고되었다(Kim & Choi-Kwon, 2000). 이러한 신체적, 정신적 변화는 뇌졸중 환자의 수면에 다양 하게 영향을 줄 수 있다. 여러 선행연구 결과, 뇌졸중 발생 후 수면-각성 주기의 변화가 자주 일어나는 것으로 보고되었다 (Leppavuori, Pohjasvaara, Kaste, & Erkinjuntti, 2002; Palomaki et al., 2003). 수면-각성 주기의 변화는 뇌졸중 환자에게서는 40-60% (Leppavuori et al., 2000) 정도가 발 생하는 것으로 보고되어, 일반인의 발생 빈도(10-40%, Hohagen et al., 1993)에 비해서 높은 것으로 보인다.
The purpose of the present study was to investigate the prevalence and related factors of sleep-wake disturbance (SWD) in Korean post-stroke patients. This study was the first to address post-stroke SWD in Korea using a structured questionnaire. Method: We investigated the prevalence of SWD and related factors including lesion location, stroke severity, presence of depression and fatigue. We assessed sleep-wake pattern including quality of sleep, sleep latency, wake episodes, wake time after sleep onset, daytime sleep episode and sleep time at daytime. Ninety stroke patients admitted to a university affiliated hospital in Seoul between the period September 2008 and January 2009 were included in the study. Result: Thirty five patients (38.9%) complained insomnia and 32 (35.6%) complained excessive daytime sleepiness (EDS). Quality of sleep (p=.000), sleep latency (p=.000) and total sleep time (p=.001) were significantly poorer in 16 patients with both insomnia and EDS than in the others. The related factors to insomnia were level of education (p=.030), depression (p=.007) and fatigue (p=.034), though related factors to EDS were stroke onset time (p=.049), stroke severity (p=.005), motor dysfunction (p=.035), dysphasia (p=.018), fatigue (p=.001) and lesion location (p=.019). Conclusion: Sleep-wake disturbance is a common problem in Korean stroke patients. Strategies to improve quality of sleep are urgently needed in the post-stroke patients.
