연구목적 본 연구는 국내 뇌졸중 환자의 상지 기능 향상을 위한 기능적 전기 자극치료 적용 연구에 대해 검 토해보고자 체계적 고찰을 시행하였다. 연구방법 2000년 1월부터 2013년 4월까지 한국교원학술정보원를 통해서 학위 논문 및 학회지에 게제된 논문 을 검색하였고 주요 검색용어로는 기능적 전기자극치료, FES, 뇌졸중 등의 단어를 사용하였다. 연구결과 각 연구의 참여한 연구대상자 수는 조사된 연구 중 2편 만이 각각 총 대상자 60명으로 가장 많아 각 군 별 대상자 수가 20이상인 연구였고 그 외 모든 연구들의 각 군 별 대상자 수는 20이하로 이 루어져 근거 수준이 높지 않았다. 각 논문의 연구설계는 무작위비교실험설계(Randomaized trial) 연 구는 없었으며, 비-무작위 비교실험설계(Non-RCT) 연구는 전체 연구 중 총 8편 이었다. 그 외 모 든 연구는 단일 집단의 사전-사후 치료의 비교연구로서 총 5편 이었다. 조사된 연구 중 2편에서의 외적 타당도는 각 연구군의 n수가 20명 이상이 확보되어 중등도의 외적 타당도 평가를 내렸으나 그 외 모든 연구들은 2의 내적 타당도와 c의 외적 타당도를 보였다. 각각의 연구별에서 얻어진 치 료 적용 효과들로서는 상지 및 손 기능 향상이 전체 연구 13편 중 12편에서 언급되어 가장 많았고 그 외 근력 향상이 6편, ROM 증가가 언급된 연구가 5편이었다. 그 외 언급되어진 기능적 전기 자 극 치료의 효과로는 대뇌피질 활성도의 변화 2편, 경직성 감소 1편, 일상생활수행능력의 변화 1편 등으로 나타났다. 결론 추후의 연구에서는 이러한 결과에 따라 보다 많은 수의 대상군의 확보와 추적조사를 통한 지속적인 치료적 효과에 대한 연구가 이루어져야 할 것이며 기능적 전기자극치료의 보다 다양한 치료적 가능 성에 대한 연구가 요청된다.
Objective : This study is executed to test research on how to apply functionally electric stimulation therapy for improving upper extremity of domestic patients. We performed systematic consideration for it. Methods : From January in 2000 to April in 2013, we had been searching for essays posted on academic thesises and journals through Korea National Research Information and using many main key-words such as functional electrical stimulation therapy, FES, stroke. Researchs : There were in total the sixty subjects in each of two trials among which had been investigated and their each group had approximately more than twenty subjects and else had less than twenty, which did not present high level of reason. There was no randomized trial in study design of each essay and there were in total the eight Non-RCT among all. Aside from them, every study was to compare pre-post therapy of single group, which were five in total. Among all which had been examined, two studies were evaluated to be reasonable because the number of n obtained more than twenty and other all studies showed two of internal validity and external validity of c. As effects gained from therapy that had been applied, 12 out of 13 studies mentioned functional development of upper extremity or hands and there was muscular improvement mentioned in six, and increment of ROM in five. For else as effect shown from functional electrical stimulation therapy, there was activity of the cerebral cortex in two, reduction of rigidity in one, change of ability to perform in daily basis mentioned in one. Conclusion : In future study, there should be constant study on therapeutic effect through investigative examination and acquirement of many groups of subject. What is more, the study on many clinical possibilities of functional electrical stimulation therapy.
