목적 : 본 연구는 치매 노인을 대상으로 지역사회 작업치료의 효과를 알아본 무작위대조군 연구에 관한 체계 적 고찰이다. 연구방법 : 2019년 1월에 PubMed 데이터베이스에서 ‘Community AND occupational therapy AND dementia’ 검 색용어로 검색하여 총 8편의 연구를 선정하였다. 문헌의 질은 Pedro scale로 분석하고, PICO로 정리하 고 연구 대상자, 중재, 대조군, 결과에 대하여 분석하였다. 연구결과 : 8편의 중재 내용을 분석하면 보조도구 사용에 대한 교육 및 훈련, ADL의 단순화에 대한 교육, 다 학문적 중재, 활동 맞춤, 인지재활이었다. 평가영역은 치매노인의 언어능력, 신체능력, 인지, ADL, 삶의 질, 신경정신학적 증상과 정서이었고, 그 중 ADL에 관한 평가가 가장 많았고, 평가도구 중에 는 IDDD를 가장 많이 실시하였다. 전체 결과값 중 유의한 향상을 보인 결과값이 그렇지 않은 결 과값보다 많았다. 결론 : 추후 치매 노인을 위한 국내 지역사회작업치료 프로그램 개발을 위한 기초자료로 활용할 수 있을 것이다.
Objective : The purpose of this study was to review systematically on randomized controlled trials studies about community based occupational therapy for the elderly with dementia. Method : Eight studies was searched by key words ‘Community AND occupational therapy AND dementia’ in PubMed database in January 2019. The quality of the literature was analyzed by Pedro scale, and the contents were summarized by PICO. Result : The intervention for the elderly with dementia showed that the education and training on the use of assistive devices and simplification of daily life activities, multifaceted intervention, activity adjustment and cognitive rehabilitation. In the evaluation, the tool of language ability, physical ability, cognition, ADL, quality of life, neuropsychiatric symptom and emotion for dementia elderly was used. The results that showed significant improvement in the all results were higher than those that were not. Conclusion : In the future, this study will be helpful as a basis for the development of domestic protocols for community-based occupational therapy programs for the elderly with dementia.
