회복지실천이 관심을 두는 다른 욕구집단 즉, 빈곤계층이나, 일반적 장애인, 혹은 범죄인 등과 마찬가지로 정신장애인들의 탈시설화가 곧 그들의 지역사회통합을 보장해 주는 것은 아니다. 오랜 기간 병원과 같은 폐쇄적인 시설에 격리되어 있던 정신장애인이 지역사회로 돌아와 통합되기 위해서는 그들의 지역사회 적응을 원조해 주기 위한 적절한 서비스가 지역사회 내에서 지속적으로 제공되어야 한다. 이러한 기능을 수행하도록 기대되고 있는 것이 바로 지역사회정신보건센터일 것이다. 2004년 6월 말 현재 우리나라에는 전국적으로 115개소의 지역사회정신보건센터가 설치되어 운영되고 있으며, 시설 이용 연 인원은 전체 등록 정신장애인의 절반에 조금 못 미치는 25,181명에 이른다. 그러나 이들은 대부분 지역사회에 온전히 통합․정착하지 못하고 시설과 지역사회를 전전하며 떠돌고 있는 실정이다. 본 연구는 이러한 현상이 효과적인 지역사회정신보건 프로그램의 부재로 인한 것이라는 문제인식으로부터 계획되었다. 정신장애인의 상당부분을 차지하고 있는 정신분열장애인의 지역사회에의 적응과 통합을 저해하는 요인을 찾아내어 적절한 해결을 도와주어야 할 필요성의 인식으로부터 본 연구는 시행되었다. 본 연구에서는 정신분열이 갖는 병리적 증상 중 사회적응능력을 결정적인 영향을 미치는 것으로 알려져 있는 음성증상과 정신장애에 대한 지역사회의 왜곡된 인식을 정신분열장애인의 지역사회통합을 저해하는 요인으로 보았다. 따라서 본 연구는 정신분열장애인의 음성증상 완화를 도울 수 있고 또 동시에 지역사회의 인식을 개선할 수 있는 프로그램을 개발하고 시행함으로써, 정신분열장애인의 지역사회통합을 위해 필요한 개인의 변화와 환경의 변화를 추구하고자 하는 목적을 설정하였으며, 이의 실천 과정 및 목적 달성 여부를 양적․질적으로 평가함으로써 향후 지역사회내에서 정신장애인들을 대상으로 계획되고 또 제공될 프로그램의 방향을 제시하고자 하였다.
Deinstitutionalization of the mental disabilities does not mean and guarantee their community integration. It is necessary that proper services are provided in the context of community to the mental disabilities who comes back to the community from highly segregated institutions like long-term care hospitals. The community mental health centers are expected for implementing the effective care service providing roles for the mental disabilities to adapt and integrate to the community. But the mental disabilities are suffering from maladaptation toward community and forcing to be re-institutionalized. This study focused on what generates these hardly adaptable situation for the mental disabilities. In general, negative symptoms of the schizophrenics and negative attitudes toward the mental disabilities which are ubiquitous in community diminish the schizophrenics\' social adaptation skills and abilities. One of the purposes of this study is developing a community mental health program which raise social adaptation abilities of the schizophrenics through improving their negative symptoms and restructuring the community attitudes toward the mental disabilities through active community participation of the schizophrenics. Perspectives and approaches adopted in this study are basically following the dual focus(person in environments) of social work practice. So, in intervention to the schizophrenics with the program which has developed for attaining the purpose of this study, efforts to synchronize micro level change(improving schizophrenics\' negative symptoms) and macro level change(restructuring community attitudes and reforming functional relationships between schizophrenics and community environment) have been kept.
