본 연구는 2008년 7월 1일부터 실시된 노인장기요양보험제도가 좀 더 효율적이고 안정적인 제도로 정착하기 위해 그 실태 및 문제점을 파악하고 개선방안을 다음과 같이 제시하고자 한다. 첫째, 저소득층노인과 정신장애 중증노인을 포함한 대상자 확대와 지역사회 중심의 요양서비스 체계구축 강화이다. 둘째, 지역사회병원과 연계한 가정간호통합시스템 및 케어매니지먼트 제도 등을 도입하여 서비스의 질적 향상 ·다양화 ·전문화를 도모하자는 것이다. 셋째, 재가시설이외 주간보호시설 확대 및 공동가정시설 등 다양한 형태의 노인장기요양시설이 갖추어져야 맞춤노인요양서비스가 이루어지며, 가족과 가까운 수도권에 많이 확대되도록 공공기관의 적극적인 참여가 필요하다. 넷째, 비공식적인 가정에서의 노인수발을 위한 ‘가족지원제도’가 마련되어야 한다.
The purpose of this study is to find the current status on the operation and to suggest the improvement plans of elderly long-term care insurance system performed from July 1, 2008. For this purpose, this study was performed with literature reviews and analysis of reported national results of the practiced system, mainly, and new plans to promote the existing system were suggested follow. First, the elderly classes with low income and severe mental diseases need to be included to the system and reinforced in community, because they are easily processed to severe elderly classes with physical and mental damages. Second, the day/night-care service and short period care service facilities to provide elderly with variate desires need to be developed as home-visiting care service. Third, the services need to be developed high quality, variate and specialize according to variate desires of elderly, as like the home-visiting nursing care management system linked to community hospitals to prove health and prevent severe disease of elderly in home. Fourth, it is necessary to make an alternative system to support burden of the family care elderly in home for reduction of severe elderly.
